DR. LEH-CHII CHWANG
ASIAN DIETETICS AWARD
Benefits:
The Award provides a cash gift of US$1,000 and a plaque courtesy of Dr. Leh-Chii Chwang, the donor.
Qualifications of the Nominee:
1. Should be a prominent dietetic professional, preferably a practitioner;
2. Has made significant contributions, locally as well as internationally, toward enhancing the dietetic profession;
3. Has actively involved in Asian Federation of Dietetic Association (AFDA) activities;
4. Has demonstrated outstanding leadership in dietetic field.
Nomination Procedure:
1. Nominations should be submitted by or come from members of the AFDA only;
2. Only one candidate should be submitted per member country or area;
3. Nomination letters should be accompanied by the following documents:
a) A letter of recommendation from the national dietetic association;
b) An updated curriculum vitae of the nominee with photo
4. Nomination documents should be sent to:

5. Nominations should be received three months prior to the Asian Congress of Dietetics (ACD)
The Selection Committee:
The Selection Committee shall be composed of the following:
1. The current President of the AFDA;
2. The immediate past president of the AFDA;
3. Dr. Leh-Chii Chwang, who will serve as the convenor of the selection process.
Note:
1. The nominator should avoid recommend one candidate for more than one award.
2. If the nominator or the nominee happens to be a member of the Selection Committee, he/she should withdraw from the Selection Committee to avoid conflict of interest. The withdrawn selection committee member should notify the Secretary General of AFDA by email and he/she should be replaced by the immediate past/or previous president.
3. The plaque, worth US$100, is to be designed and prepared by the host Organizing Committee.

ASIAN OUTSTANDING YOUNG DIETITIAN AWARD
Benefits:
The Award provides a cash gift of US$1,000 and a plaque courtesy of Dr. Ching-Hwa Chiu, the donor.
Qualifications of the Nominee:
1. Should be a dietetic professional, aged below 45;
2. Has demonstrated his/her potentiality in nutrition and dietetics, through active participation and extensive service, domestically and internationally,
3. Be a promising dietetic leader
Nomination Procedure:
1. Nominations should be submitted by or come from members of the Asian Federation of Dietetic Association (AFDA) only;
2. Only one candidate should be submitted per member country or region;
3. Nominations should be accompanied by the following documents:
a) A letter of recommendation from the national dietetic association;
b) An updated curriculum vitae of the nominee with photo;
c) A photocopy of ID card or other documents of the nominee, indicating the date of birth
4. Nomination documents should be sent to:
